A. AFFIDAVIT
Requirements
1. Personal appearance of affiant/s
2. Presentation of at least one current identification document, such as a valid Philippine Passport, or any Philippine government-issued identification card (driver's license, professional license, SSS ID, Pag-Ibig ID, POEA ID)
3. Application fee: €25.00
Release date: One (1) day after filing of application.
Note: Affidavit of Consent (for the travel of a minor child) shall be executed only if neither parent will accompany the child
Exception: even if unaccompanied by parent/s, a Travel Clearance is not required for a minor whose parents are living abroad or are immigrants - minor should be holding a valid pass: dependent's visa/ pass/ identification card or permanent resident visa /pass /identification card

B. SPECIAL POWER OF ATTORNEY, DEEDS & OTHER DOCUMENTS REQUIRING ACKOWLEDGMENT
Requirements
1. Personal appearance of the principal/s or signatories
2. Presentation of at least one current identification document, such as a valid Philippine Passport, or any valid Philippine government-issued identification card (Driver's license, SSS ID, Pag-Ibig ID, POEA ID)
3. Application fee: €25.00
Release date: Three (3) working days after filing of application.

C. AUTHENTICATION
Requirements
1. Presentation of documents with the original stamp of the Procura della Repubblica, Prefettura or the Camera di Commercio in places under the jurisdiction of the Philippine Consulate General
2. Documents may be filed by an agent, or sent through a courier. (If the sender would like to have his documents returned through courier service, he should enclose a self-addressed envelope and a duly accomplished prepaid airway bill.)
3. Application fee:€25.00
Release date: Three (3) working days after filing of application.

D. AMENDMENT OF REPORT OF BIRTH
(due to subsequent marriage or acknowledgment by the father)
*Only for births registered at the Philippine Consulate General in Milan
Requirements
1. Original or authenticated copy of the report of birth
2. Authenticated marriage contract (for children legitimated by subsequent marriage of the parents)
3. Affidavit of Acknowledgment duly executed by the father (for children who were acknowledged by the father)
4. Application fee:€25.00
Release date: Three (3) working days after filing of application.

E. CERTIFICATE OF RESIDENCY
(for travel tax exemption purposes)
*For members of religious communities
Requirements
1. Personal appearance of the applicant
2. Letter from the congregation certifying that the applicant is a member thereof and his/her continuous residence in Italy
3. Current passport of applicant and photocopy thereof
