Consular Services

I. STANDARD REQUIREMENTS FOR PASSPORT APPLICATIONS

A. Passport RENEWAL
1. Latest passport (original) and photocopy of its data page and amendment page, if applicable
2. Duly accomplished application form
3. Application fee of €60.00
4. Other documents as may be required such as Permesso di Soggiorno or Carta d'Identita, if applicant is documented
5. For married women adopting the surname of her husband, authenticated Marriage Contract (original and photocopy)
6. For married women who intend to revert to their maiden name, authenticated Death Certificate or Marriage Contract with Annotation (original and photocopy), whichever is applicable.

Note: Women who are legally separated from their husbands cannot revert back to their maiden name.

B. Passport Application : NEWBORN / FIRST-TIME APPLICANT
1. Duly accomplished application form
2. Application fee of €60.00

C. Replacement of LOST PASSPORT
1. Duly accomplished application form
2. Application fee of €150.00 (for lost ePassport) or €90.00 (for lost MRRP or MRP)
3. Authenticated Birth Certificate, original and photocopy
4. For married women, authenticated Marriage Contract, original and photocopy
5. Affidavit of Loss, with fee of €25.00 (for undocumented nationals)
6. Police Report/Denuncia, for holders of Permesso di Soggiorno
7. Other documents as may be required, e.g., Permesso di Soggiorno, Driver's License,
PRC/SSS/COMELEC ID
Note: A 15-day clearance period is required for application for the replacement of lost passport.

IMPORTANT REMINDERS

  • The release date of passports will be indicated in the applicant's official receipt and is usually due four (4) weeks after the date of application.
  • Passports shall only be released to the applicant or his/her representative (with proper identification and written authorization signed by applicant). Exemptions are applicants below 18 years old whose passport can be claimed by the parent/guardian who submitted and signed the application. To claim the passport, bring the previous passport and official receipt, and authorization (for representative claimants). No passport shall be released without the said documents.
  • The Consulate General only accepts birth certificates/marriage contracts/death certificates issued by the National Statistics Office and duly authenticated by the Department of Foreign Affairs.




II. EXTENSION OF PASSPORT VALIDITY
The DFA has allowed the extension of the validity of the MRRP (green passports) and MRP in emergency cases such as immediate travel to and from the Philippines or the renewal of residency permits. Only passports with an expired validity or valid for less than six (6) months, can have their validity extended. The validity of passports can only be extended once. Applicants for passport extensions must also apply for the renewal of their passport simultaneously.

III. APPLICATION FOR TRAVEL DOCUMENT
A Filipino citizen who does not have a valid passport, for one reason or another and wishes to return to the Philippines, may apply for a travel document. This document is valid for only a one-way travel to the Philippines.

Requirements
1. Duly accomplished application form
2. Application fee of €30 .00
3. Affidavit of explanation
4. Three (3) identical colored photographs, size 3.50 cm x 4.50 cm
5. Latest passport or Authenticated Birth Certificate
6. Other documents as may be required
