A. FOR CHILDREN WHOSE PARENTS ARE MARRIED
Requirements:
1. Duly accomplished Report of Birth signed by both parents
2. Application fee: €25.00
3. Birth Certificate (international version) issued by the Comune (original and 3 photocopies) indicating the place and date of birth and the names of both parents.
4. Passport of both parents (original and 4 photocopies)
5. Authenticated Marriage Contract (original and 4 photocopies) or Contract of Marriage (if married at the Consulate General)
B. FOR CHILDREN WHOSE PARENTS ARE NOT MARRIED & WHO WILL ADOPT THE FATHER'S SURNAME
Requirements:
1. Duly accomplished Report of Birth signed by both parents
2. Application fee: €25.00
3. Duly accomplished Affidavit of Acknowledgement/ Admission of Paternity/ to Use the Surname of the Father or hand written acknowledgement if the father is not a Filipino citizen – 4 sets
4. Birth Certificate (international version) issued by the Comune (original and 3 photocopies) indicating the place and date of birth and the names of both parents.
5. Passport of both parents (original and 4 photocopies)
C. FOR CHILDREN WHOSE PARENTS ARE NOT MARRIED & WHO WILL ADOPT THE MOTHER'S SURNAME
Requirements:
1. Duly accomplished Report of Birth signed by the mother
2. Application fee: €25.00
3. Birth Certificate (international version) issued by the Comune (original and 3 photocopies) indicating the place and date of birth and the names of both parents.
4. Passport of the mothers (original and 4 photocopies)
D. FOR CHILDREN WHOSE PARENTS ARE NOT MARRIED BUT WHOSE MOTHER IS MARRIED TO ANOTHER MAN
Requirements:
1. Duly accomplished Report of Birth signed by the mother
2. Application fee: €25.00
3. Duly accomplished Affidavit of Illegitimacy of a Married Woman (separate payment of €25.00) – 4 sets
4. Duly accomplished Affidavit of Acknowledgement/ Admission of Paternity/ to Use the Surname of the Father if the child will adopt the father's surname (separate payment of €25.00) – 4 sets
5. Birth Certificate (international version) issued by the Comune (original and 3 photocopies) indicating the place and date of birth and the names of both parents.
6. Passport of both parents (original and 4 photocopies)
REMINDER: Birth of Filipino citizens abroad must be immediately reported to the Philippine Embassy or Consulate General. Applications for Report of Birth after 1 year from the birth of the child shall require the execution and submission of an Affidavit of Delayed Registration.
